OUR FEES Effective February 1st, 2012

New York House Call Physicians do not have any hidden fees. If you are

unclear about any medical fees, please call 646.957.5444 and ask. [Please

note: No Medicare and No Medicaid Accepted but discounts offered to some

Medicare and Medicaid patients. Some PPO health insurance plans accepted.

Below is a list of our fees:

Cancellation Fee=100% of the consultation cost [A "cancellation fee" will

be applied to all appointments cancelled within 24 hours of the

appointment request]

No Show Fee=100% of the cost of your visit [A "no show fee" is when you

have a doctor visit scheduled--home or office--and you are not there when

the doctor is in the office for your scheduled visit; or is en route to

your scheduled house call; or is at your house call location and, for

whatever reason, the house call does not occur]

Doctor Office Visit=$450

Doctor Hospital Visit=$900

Doctor House Call (NYC except Staten Island)=$900

Doctor House Call (Staten Island)=$1200

Doctor House Call (outside NYC)=$900 plus $450 per hour transit time (in

each direction)

Doctor Telephone Consultation=$450

Intravenous Therapy Office Visit=$300

Intravenous Therapy House Call (NYC except Staten Island)=$600

Intravenous Therapy House Call (Staten Island)=$800

Intravenous Therapy House Call (Outside NYC)=Please call to confirm fees

On Call Fee [Doctor]=$225 per hour

On Call Fee [Nurse or Medic]=$90 per hour

Medic or Nurse House Call [home phlebotomy and/or urinary bladder

catheterization] (NYC except Staten Island)=$450

Medic or Nurse House Call [home phlebotomy and/or urinary bladder

catheterization] (Staten Island)=$550

Medic or Nurse House Call [home phlebotomy and/or urinary bladder

catheterization] (outside NYC)=Please call to confirm fees

Physical Therapy (Manhattan)=$400

Physical Therapy (Outside Manhattan)=Please call to confirm fees

Prolonged Service Fee=50 to 100% of the consultation cost [A "Prolonged

Service Fee" is when the patient is late for their appointment, requiring

New York House Call Physicians to wait greater than 30 minutes after the

scheduled appointment time or if a consultation is protracted]

Psychotherapy Office=$250

Psychotherapy House Call (NYC)=$500

Ultrasound Office=$600 per body area

Ultrasound (NYC except Staten Island)=$600 per body area

Ultrasound (outside NYC)=Please call to confirm fees

Vaccine=$75 to $200 [please call to confirm vaccine fees as the cost &

availability of vaccines vary]

X-Ray Office=Not available

X-Ray Home (NYC)=$450 per body area

X-Ray (outside NYC)=Please call to confirm fees
